چکیده

Background: Diabetic ketoacidosis (DKA) is an acute metabolic healthcare crisis in patients with diabetes mellitus. The current study aimed to compare the effectiveness of rapid-acting insulin analog administered subcutaneously regular infused intravenously among DKA patients. Methodology: In this prospective open labelled study, 100 consecutive were randomly assigned two groups. Group 1 admitted intensive care unit (ICU) and treated intravenous infusion. 2 managed emergency medical ward subcutaneous insulin. Response therapy was assessed by follow-up investigations biochemical parameters, including blood glucose concentration, serum ketones, pH, electrolytes bicarbonates, sodium potassium concentration until resolution DKA. Furthermore, overall duration (blood level < 250 mg/dl), time amount DKA, also assessed. Results: The baseline clinical parameters similar between treatment groups except for concentration. mean random sugar (RBS), acid-base ketone bodies significantly improved from admission There no significant difference (p=0.07). While required reduced insulin, i.e. 16.36 ± 6.92 hrs 59.28 30.05 units (p<0.05). Conclusion: less complicated can be wards obviating need ICU. With relatively better outcomes, it effective alternative infusion resolution.

Insulin Analogs Versus Human Insulin in the Treatment of Patients With Diabetic Ketoacidosis

OBJECTIVE To compare the safety and efficacy of insulin analogs and human insulins both during acute intravenous treatment and during the transition to subcutaneous insulin in patients with diabetic ketoacidosis (DKA). RESEARCH DESIGN AND METHODS In a controlled multicenter and open-label trial, we randomly assigned patients with DKA to receive intravenous treatment with regular or glulisine ...

Vascular effects of rapid-acting insulin analogs in the diabetic patient: a review

The insulin analogs lispro, aspart, and glulisine are the only commercially available rapid-acting insulins to treat diabetes. We review the evidence for treating hyperglycemia, using insulin, and specifically using rapid-acting analogs in diabetic individuals, on the prevention of vascular events. We review the beneficial effects of insulin on the vascular system, which include vasodilation an...

Management of Diabetic Ketoacidosis in Severe Insulin Resistance

Syndromes of severe insulin resistance (IR) include mutations of or autoantibodies to the insulin receptor and lipodystrophy (1). Diabetic ketoacidosis (DKA), although rare, can occur in these patients, even in the context of hyperinsulinemia, due to impaired insulin signaling. DKA can be extremely challenging to treat, and few clinicians are experienced or comfortable in using the high doses o...

Long-Acting Human Insulin Analogs

Endogenous insulin is a hormone secreted by the beta cells of the pancreatic islets of Langerhans. Insulin acts via specific membrane-bound receptors on target tissues to regulate metabolism of carbohydrates, proteins and fats. Exogenous insulin, manufactured for pharmacologic use through recombinant DNA technology using either E. coli or Saccharomyces cerevisiae is used as replacement therapy ...

Long-Acting Insulin Analogs and the Risk of Diabetic Ketoacidosis in Children and Adolescents With Type 1 Diabetes

OBJECTIVE To investigate if long-acting insulin analogs decrease the risk of diabetic ketoacidosis (DKA) in young individuals with type 1 diabetes. RESEARCH DESIGN AND METHODS Of 48,110 type 1 diabetic patients prospectively studied between 2001 and 2008, the incidence of DKA requiring hospitalization was analyzed in 10,682 individuals aged /=2 years. RE...
